I acknowledge I have read the cancellation policy carefully and agree to it's terms:
• if more than 15 minutes late, I am not guaranteed my appointment and it may be considered cancelled.
• If I need to reschedule, I will provide at least 7 days notice in order to transfer the booking fee to the new date, otherwise it is it is non-refundable.
• If sick leading up to the appointment please provide as much warning as possible in order to reschedule your appointment and transfer your deposit*
