Tattoo Waiver

I am not and will not be under the influence of alcohol or drugs for my appointment.*

I do not have major acne, moles, sunburn, rashes in the area to be tattooed that might be agitated by the tattoo process.*

I have looked over my design, checked the spelling if applicable, and give my full consent to the application of my tattoo.*

I have reviewed the aftercare requirements (gratitudetattoos.ca/aftercare) and am committed to following the protocols outlined*

I acknowledge that I am not pregnant. *

I acknowledge that I am free of communicable disease. *

I am 18 years or older OR have my parent/guardian with me who is providing their consent.

I am undertaking this procedure at my own free will.*

I acknowledge that tattooing is a permanent change to my appearance and that no representations have been made to me as to the ability to later change, alter or remove my tattoo*

I acknowledge it is not reasonably possible to determine whether I might have an allergic reaction to the dyes, pigments, or processes used in my tattoo and I agree to accept that such risks are possible*

I acknowledge that infection is always possible as a result of obtaining a tattoo particularly in that event that I do not take proper care of my tattoo. I understand the signs and symptoms of infection and will inform my tattoo artist of any signs of infection and seek medical care if necessary*

I acknowledge receipt of verbal instructions AND that I have read the aftercare instructions advising me of proper care of my tattoo and recognize the absolute necessity of following those instructions. All questions about the body art procedure have been answered to my satisfaction*

I acknowledge that variations in color and design may exist between any tattoos as selected by me and as ultimately applied to my body*

I acknowledge there is a chance of complications to the tattoo such as “blow out”. Tattoo blowout happens when ink from a new tattoo heals improperly and spreads unevenly into the skin areas outside the lines of the tattoo. They are sometimes referred to as ink bleeds or tattoo bruising*

I acknowledge that there is a chance I might feel lightheaded, dizzy during or after being tattooed. I agree to immediately notify the practitioner in the event I feel lightheaded, dizzy and/or faint before, during or after the procedure*

I agree to follow all instructions concerning the care of my tattoo, and that any touch-ups needed because of my own negligence will be done at my own expense. I understand that I get ONE free touch up and then must pay for subsequent touch ups*

I acknowledge I have read the cancellation policy carefully and agree to it's terms:

• if more than 15 minutes late, I am not guaranteed my appointment and it may be considered cancelled.

 • If I need to reschedule, I will provide at least 7 days notice in order to transfer the booking fee to the new date, otherwise it is it is non-refundable. 

• If sick leading up to the appointment please provide as much warning as possible in order to reschedule your appointment and transfer your deposit*

I agree to release and forever discharge and forever hold harmless Marni McConnach,  Gratitude Tattoos, The Yurt and its associates, agents officers and shareholders from any and all claims, damages, or legal actions arising from or connected in any way with my tattoo or the procedures and conduct used to apply my tattoo and any and all tattoos applied by Marni McConnach and its associates, agents and representatives in the future. *

I consent to the above points, am informed of the tattoo process and I assume any and all risks that may arise from this tattoo

I declare that the info I’ve provided is accurate and complete.*
